A Clinical Study on the Cause of Gastric Perforation


Abstract
The author performed a clinical analysis on 24 cases of gastric perforation on the aspect of it's cause. the clinical material was collected from 293 cases of gastro-intestinal tract perforations in the Department of Surgery, Seoul Red Cross
Hospital
from Jan. 1987 to Dec. 1992.
@ES The following results were obtained:
@EN 1) Common causes of gastric perforations were benign ulcer(50%), physical abdominal trauma(25%), adenocarcinoma(2.08%) and chemical trauma(4.2%) in order of frequency.
2) The average age of gastric perforations was at the age of 45.9 and the difference of average age was prominant in each cause. like as 29.4 in trauma, 49.3 in benign ulcer and 60.8 in adenocarcinoma group.
3) The male to female ratio was 3:1
4) The perforations were occurred commonly at the anterior wall of stomach(82.6%). The main perforation sites were the iesser curvature side between pylorus and incisura angularis(69.2%) in benign ulcer, fundus and cardia(75%) in adenocarcinoma
and
the
greater curvature side(66.7%) in trauma group.
5) Giant ulcers, greater than or equal to 3 cm, were 69.2%. The risk of the presence of microscopic maligancy in the benign-looking giant ulcer was 11.1%.
6) The operative motality was 20.8% But, there was no motality in the stab wounded.
